Be sure to read the instructions for the snack and shirt fees as well. (Please register each year so we can keep accurate phone and emergency information for each child.)

If your child is in KISD schools and you want the school bus to take your chld(ren) to St. Luke's on Wednesdays, please also do the following:

Pick up a KISD Transportation Form for each child either from the church or the school. The school needs one permission form per child needing to be picked up. St. Luke's will have the forms available at the Sanctuary exits, the church office, and in James Vermillion's office. Each school's office will also have them.
Fill out the KISD transportation form(s) and take it to the church office or to the school your child attends. The school must have the form before the child can ride the bus.

If your child is in Sabine ISD schools and you want the church van to take your child to St. Luke's on Wednesdays, please do the following:

Download the Church Van Permission Form and print one out for each child (there are two per page already for your convenience). (The permission form is in PDF format which can be viewed in Adobe Reader®. If you need Adobe Reader®, you can click on the link for it at the bottom of this page.)
Fill out one form for each child and return it to the church office. We cannot pick up your child from a Sabine school until we have the permission form in hand. We will take care of giving the permission forms to the schools. We will keep a copy in our files as well.
